          Description

          This booklet, which summarizes important lessons learned from the analysis of nuclear incidents worldwide over the period 1999-2002, is a joint publication of the International Atomic Energy Agency (IAEA) and the OECD Nuclear Energy Agency (NEA). Several areas from the reports of the Incident Reporting System (IRS) have been selected in order to obtain different types of failure in a variety of plant systems, as well as human performance considerations.
          The booklet is written both for a non-technical audience and for senior officials in industry and government who have decision-making roles in the nuclear power industry.
          The first booklet, which covered the period July 1996–June 1999, was widely acclaimed and encouraged both agencies to prepare a second one.
